Thank you for your interest in Mid-Del Group Home, Inc. Enclosed you will find eligibility criteria and our application for services. This application will serve our needs for vocational and/or residential services.

To begin the application process the applicant must meet the following criteria.
1. Applicant must be at least 18 years of age.
2. Applicant must have a Primary Diagnosis of Mental Retardation.
3. Written verification of D.D.S.D. eligibility.

The following information is also needed to process the application.
1. A psychological evaluation stating that the applicant’s primary disability is mental retardation. The
  evaluation has to have been completed within three years from date of application.
2. Most current medical exam/evaluation
3. Applicant needs to complete the Social Development Summary that is enclosed with this packet.
4. Medication and Immunization Record
5. Guardianship document (if applicable)

The following information will need to be provided once the applicant’s application has been processed and before the applicant can be considered for placement.
1. A certified copy of the applicant’s birth certificate
2. A copy of the applicant’s Social Security card

After this part of the application process has been completed, call to set up an appointment. At this time we will discuss the services we provide, our hours of operation and to answer any questions that you might have.
